Keep reading...A day in the life of a Case Management Supervisorat The Iowa Clinic : Provides oversight of the day-to-day operations of certain areas of the Population Health Team.Assist Director of Population Health with oversight, supervision, and day to day operations. Conducts process improvement, problem resolution, and answers general staff questions.Assists the Director of Population Health in leading a team of Clinical Care Managers, Diabetes Educators and Clinical Data Abstractor with the focus on value-based care.Works closely with the Director of Population Health to identify, develop, and implement care management and quality initiatives to improve the health of identified populations, reduce utilization, and improve cost effectiveness. Provides adequate training and information to new and existing employees. Includes being a resource for general questions and issues. Manages timecards and PTO requests. Participates in hiring, provides training, onboards new team members, evaluates performance, disciplines staff as needed and completes annual reviews. Collaborates with all members of the multi-disciplinary healthcare team to ensure the delivery of high quality and cost-effective care, evidenced by achievement of quality outcome metrics within our value-based contracts.Assumes accountability for enhancing own knowledge through professional associations, journals, workshops, in-services, etc. Keeps professional license current.
