Job Description :
We are seeking a strategic and results-driven leader to oversee Care Management, Care Coordination, and Clinical Transformation. This role will be responsible for developing and implementing strategic initiatives, optimizing operational workflows, and ensuring the delivery of high-quality, patient-centered care. The Director will lead a team of professionals, manage financial performance, and collaborate with key stakeholders to enhance patient outcomes and streamline care processes.
Responsibilities :
Strategic Leadership :
- Develop and implement a comprehensive strategic vision for Care Management, Care Coordination, and Clinical Transformation, aligning with the company's overall strategic goals.
- Identify and analyze industry trends, best practices, and emerging technologies to inform strategic decision-making.
Operational Excellence :
Oversee the day-to-day operations of the Care Management, Care Coordination, and Clinical Transformation teams, ensuring seamless integration and efficient workflow.Establish and maintain robust care management and care coordination processes, including patient identification, assessment, care planning, and ongoing monitoring.Team Management & Development :
Position will have 5 direct reportsLead, mentor, and develop a high-performing team of care managers, care coordinators, clinical specialists, and other professionals.Foster a collaborative and supportive work environment that encourages innovation, professional growth, and high levels of engagement.Recruit, onboard, and retain top talent to build a world-class team.Clinical Excellence & Patient Experience :
Ensure the delivery of high-quality, patient-centered care through effective care management and coordination.Develop and implement programs to improve patient satisfaction, engagement, and adherence to care plans.Collaborate with clinical staff to optimize care pathways and ensure adherence to clinical guidelines and best practices.Financial Management :
Oversee the department's budget, ensuring efficient resource allocation and financial performance.Identify and implement cost-saving initiatives while maintaining high-quality care.Track key performance indicators (KPIs) and provide regular reports to senior management on the department's performance.Stakeholder Management :
Build and maintain strong relationships with key stakeholders, including internal and external partners, physicians, nurses, patients, and families.Collaborate effectively with other departments to ensure seamless integration of care management and coordination services.Requirements :

Must work onsite 2-3 days per week.Must be local to Pittsburgh, PA.Master’s degree in nursing (MSN), Health Administration (MHA), or a related field required.Minimum of 10 years of proven experience in a senior leadership role within care management, care coordination, or clinical transformation.Deep understanding of care management principles, care coordination models, and clinical transformation strategies.Strong track record of success in developing and implementing innovative care delivery models.Proven ability to lead, motivate, and develop high-performing teams.Excellent communication, interpersonal, and presentation skills.Strong analytical and problem-solving skills with a data-driven approach.Passion for improving patient outcomes and enhancing the quality of care
