Nurse Practitioner Job Summary:
JPCHC is seeking a Family Medicine Nurse Practitioner for the Greenfield Jane Pauley CHC. The Nurse Practitioner conducts comprehensive health assessments, demonstrates a high level of autonomy and expert skill in the diagnosis of complex health responses of individuals, families, and community to actual or potential health problems. The Nurse Practitioner formulates clinical decisions to manage and promote wellness. The Nurse Practitioner integrates education, research, management and leadership in a clinical role and functions in a collegial relationship with other health care professionals to ensure continuity of healthcare.
This position provides the opportunity to serve an established panel of patients in a medically underserved community. The provider will have the support of in-house wrap-around services including Outreach Specialists, Care Managers, and Patient Care Coordinators. JPCHC is a collaborative office known as a pillar of care in Hancock County.
Education and/or Experience:
- Master of Science Degree in Nursing from an accredited School of Nursing with National Certification as an Advanced Practice Nurse.
- Current Indiana Nurse Practitioner License and current Indiana Registered Nurse License.
- Three to Five (3-5) years experience of Family Medicine experience as a Nurse Practitioner
- Current CSR and DEA certification
- Current BLS certification
- Educational background consistent with requirements for licensure and certification within the state of Indiana
- Demonstrate a high level of expert skill in delivering exceptional quality of care while enhancing the wellness of the patient
