Job Summary: Outreach, RN
The outreach RN works closely with the quality team and JPCHC service locations to provide assessment, care coordination, education, and support services to individuals at risk for or living with preventable illnesses, infections, or chronic conditions. The RN will assist people with connection to and retention in specialty care, primary care, and other services as needed
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Work collaboratively with the different departments of the JPCHC organization to help identify clients vulnerable to preventable illnesses and infections.
- Provide clinical assessments for individuals at risk of illness, referred for services, or identified through outreach.
- Work in collaboration with medical doctors, advanced practice providers, behavioral health providers, and clinical pharmacy to provide harm reduction interventions, adherence support, health education, and clinical monitoring and follow up.
- Conduct point of care screenings for illnesses and infections of public health significance and provide kits for home testing and instruction on use as appropriate.
- Collect specimens for laboratory-based testing and prepare for sending to lab.
- Conduct nursing assessment and triage for patients.
- Assist patients through healthcare system by acting as patient advocate.
- Maintain a high level of confidentiality while interacting with patients and clients with empathy and respect.
- Complete necessary forms and reporting.
- Participate in outreach and education programs for the community, such as health fairs, screenings, classes, and other events during traditional and non-traditional hours.
- Participate and lead quality improvement initiatives.
- Document patient encounters timely, consistently, and thoroughly in EMR.
- In conjunction with the quality and clinical education teams, provide training and serve as a resource to JPCHC care team members.
- Assist with special projects and other duties as necessary.
Supervisory Responsibilities
- N/A
Education and/or Experience Required:
- Graduation from an accredited nursing program
- Current state licensure as a Registered Nurse
- BLS Certification
- Experience with community outreach in vulnerable populations
- Knowledge of the Indianapolis area neighborhoods and community resources
- Point of care lab testing certifications required. May be obtained within 3 months of hire
- Must be willing to travel to different locations
- Valid Indiana Drivers License and proof of auto insurance required
Experience Preferred
- Bi-Lingual/Multi-Lingual in Spanish or Haitian Creole.
- Experience with community outreach in vulnerable populations.
- Experience working with adolescents and young adults.
- Two years of previous experience in primary care, public health, or sexual health/family planning settings preferred.

- The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and ability to adjust focus.
