Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex systems, ensuring they receive the necessary resources and services to improve their overall well-being. This position requires a strong understanding of behavioral health, crisis intervention, and workforce development, along with the ability to work collaboratively with clients from diverse backgrounds.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop individualized service plans.
- Provide ongoing case management support, including regular follow-ups and progress monitoring.
- Facilitate access to community resources such as housing assistance (including Section 8), healthcare services, and workforce development programs.
- Utilize motivational interviewing techniques to empower clients in setting and achieving personal goals.
- Coordinate care among various service providers, ensuring a holistic approach to client support.
- Implement crisis intervention strategies when necessary, offering immediate support and guidance during critical situations.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes in compliance with agency policies.
- Supervise interns or junior staff members as needed, providing mentorship and guidance in case management practices.
Qualifications
- Bachelor's degree in social work, psychology, or a related field; relevant experience may be considered in lieu of a degree.
- Experience in behavioral health settings is preferred; knowledge of addiction counseling is a plus.
- Familiarity with Medicare processes and regulations is advantageous.
- Strong skills in crisis management and intervention techniques.
- Experience working within child welfare systems is beneficial.
- Excellent communication skills, both verbal and written, with the ability to build rapport with clients effectively.
- Ability to work independently as well as collaboratively within a team environment.
- Proficiency in using case management software and maintaining accurate records.

Pay: $36,000.00 - $40,000.00 per year
