Job Summary
We are seeking a Care Coach to assess and support members in achieving optimal health and wellness. This role involves home, facility, and remote visits to guide members and their families toward appropriate care resources. The ideal candidate will have experience in case management, care coordination, and community health resources.
Key Responsibilities
- Assess and evaluate members’ needs to develop personalized care plans.
- Identify and resolve barriers to effective care management.
- Continuously monitor patient progress through assessments, data analysis, and member interactions.
- Educate and guide members on available healthcare and community resources.
- Maintain accurate electronic case notes and document within multiple systems.
- Follow established regulations, standards, and professional guidelines in case management.
- Bachelor’s degree (Required).
- 2 years of related experience in healthcare, case management, or care coordination.
- Proficiency in Microsoft Office (Word & Excel).
- Strong verbal, written, and interpersonal communication skills.
- Ability to use multiple electronic applications and software programs.
- Must pass Tuberculosis (TB) screening (role involves patient-facing responsibilities).
- Valid driver’s license and proof of personal vehicle liability insurance
- Passion for improving consumer healthcare experiences.
- Master’s degree in a related field.
- Experience working with Medicare & Medicaid recipients .
- Prior experience with electronic case note documentation and multi-system data entry.
- Background in health promotion, coaching, and wellness .
- Knowledge of community health and social service agencies .
