Description

Our client is seeking an experienced and dedicated Counselor to join their team in Martinsville, Indiana! If interested and qualified, please apply immediately!

Summary: The Counselor oversees all aspects of a patient’s treatment, ensuring coordination and continuity of care from admission through discharge and follow-up. This role also promotes performance improvement and service delivery by providing comprehensive psychological therapy to address the causes and effects of addiction, including substance abuse counseling for assigned patients, family, and group therapy.

Essential Functions

  • Interface with the clinical team in decision-making processes involving patients, including attending all clinical meetings.
  • Assess the counseling needs of patients.
  • Diagnose substance abuse using DSM criteria and determine the appropriate level of care based on ASAM criteria.
  • Obtain relevant previous records for the current treatment episode.
  • Prepare the substance abuse counseling portion of the patient treatment plan.
  • Provide substance abuse counseling services as specified in the patient treatment plan.
  • Develop patient discharge/continuum of care plans to ensure appropriate levels of care.
  • Participate fully as a member of a multidisciplinary team for assigned patients.
  • Document all counseling and education services, assessments, reassessments, referrals, and follow-up in the patient’s clinical record.
  • Maintain patient caseload, including case management, utilization review updates, treatment planning duties, and clinical charting.
  • Coordinate support services for patients and their families, recording services provided in the clinical record.
  • Reassess patients and revise the substance use counseling portion of the treatment plan as necessary.
  • Facilitate family therapy as assigned.
  • Contact and coordinate with referral sources (e.g., legal, school, employer).
  • Facilitate intervention for crisis-oriented issues using a multidisciplinary team approach.
  • Perform administrative and clinical functions as requested.
  • Attend team meetings and complete all required training courses timely.

  • Ability to utilize a wide variety of office technology and tools to collect, enter, process, and file patient records and documentation.

Education

  • Bachelor’s Degree required.
  • Counselors must be credentialed under the Department of Mental Health and Addiction (DMHA) as one of the following: Licensed Clinical Social Worker (IC 25-23.6-5) with an addiction credential, Licensed Mental Health Counselor (IC 25-23.6-8.5) with an addiction credential, Licensed Marriage and Family Therapist (IC 25-23.6-8) with an addiction credential, Licensed Clinical Addiction Counselor (IC 25-23.6-10.5), Psychologist (IC 25-33), Physician (IC 25-22.5-2), Advanced Practice Nurse or Certified Nursing Specialist (IC 25-23-1), or credentialed in addictions counseling by a nationally recognized credentialing body approved by the division, such as: Licensed Addiction Counselor (LAC), Certified Alcohol and Drug Addiction Consultant II (CADAC II), Certified Alcohol and Drug Addiction Consultant III (CADAC III), Certified Alcohol and Drug Addiction Consultant IV (CADAC IV), Certified Alcohol and Drug Addiction Consultant V (CADAC V), Addiction Professional in Training (APIT), Certified Addiction Counselor Level I (ICACI), Certified Addiction Counselor Level II (ICACII), National Certified Addiction Counselor, Level I (NCAC I), National Certified Addiction Counselor, Level II (NCAC II), Master Addiction Counselor (MAC).
