Position Summary : The Director of Discharge Planner is responsible for coordinating and facilitating the discharge process for patients within the facility. The Discharge Planner ensures a smooth transition from the nursing home to the patient’s next level of care, whether that is home, an assisted living facility, or another healthcare setting. This role involves comprehensive discharge planning, community service integration, and referrals to appropriate post-discharge services to support optimal patient outcomes. The Discharge Planner will work closely with Social Services and the Interdisciplinary team to ensure an interdisciplinary approach.
Qualifications & Requirements :
- Minimum of 2 years of experience in discharge planning, case management, or a similar role within a healthcare setting.
- Bachelors Degree in Nursing, Social Work, or a related healthcare field (Preferred)
- Knowledge of clinical services and insurance coverage, discharge planning processes, and community resources preferred.
