Job Description
We are seeking a skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality patient care, conduct comprehensive assessments, and develop individualized treatment plans.
Key Responsibilities:
- Conduct in-home visits and community-based services to patients with chronic health conditions
- Perform thorough assessments, including medical history reviews and physical examinations
- Diagnose and treat patients using evidence-based practices and guidelines
- Manage and monitor chronic health conditions, such as diabetes and hypertension
- Prescribe medications and educate patients on proper administration
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
Required Skills and Qualifications
To be successful in this role, you must have:
- A current, unrestricted licensure as a Nurse Practitioner in the state where you will be providing care
- Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
- A strong understanding of risk adjustment models for CMS and the Affordable Care Act
- Excellent communication and patient education skills
- Proficiency in electronic medical record systems
