Nurse Practitioners will be traveling to patient homes within the county limits to complete Annual Health Assessments.
The assessments will include the following responsibilities :
- Reviewing patient's past medical history
- Review of systems
- Physical Exam
- Medication Review
- Depression Screening
- Checking Vitals
- Formulating list of current and past medical conditions
