About Us: Join our leading provider of risk adjustment, quality improvement, and value-based solutions for health plans and providers. We are expanding rapidly and seeking a Certified Nurse Practitioner (NP) to conduct in-home wellness risk adjustment assessments for Medicare members and at-risk populations.
Key Responsibilities:
Conduct annual in-home wellness visits and health assessments for members with chronic conditions.
Perform comprehensive evaluations, including physical exams, vital sign assessments, and medication history reviews.
Assess members’ functional status, cognitive abilities, home environment, and more.
Provide patient health education on topics like pain management.
Assist in closing care gaps and facilitate screenings.
Build trust through exceptional communication and empathy.
Collaborate to enhance care delivery.
Qualifications:
Valid NP license in the state of practice.
Access to reliable transportation within a 55-mile radius.
Preferred: Experience in in-home risk assessments, patient care experience, EMR proficiency, bilingual skills.
