Location: Hybrid (mostly remote) - Must reside in the Baltimore area for periodic office visits
Schedule: Monday to Friday, 8 AM - 5 PM
Compensation: $46 per hour
Job Description:
The RN Coordinator serves as the primary contact for patients, ensuring seamless coordination and delivery of services. This role involves educating patients about healthcare options, addressing their questions, and fostering trust. The RN Coordinator collaborates with healthcare providers and staff to manage schedules, appointments, and patient needs, ensuring efficient access to information and task delegation within the team.
Key Responsibilities:
- Conduct telephonic nursing assessments and triages based on established protocols.
- Oversee all aspects of patient care, including scheduling appointments, obtaining prior authorizations, and managing overall health.
- Facilitate communication between providers and patients, delegating tasks as necessary.
- Educate patients on care options and provide informed recommendations.
- Explain medical test results, diagnoses, and treatment plans to patients or caregivers.
- Identify care gaps and intervene appropriately.
- Coordinate services, educate patients, and participate in HEDIS initiatives.
- Enhance access to care and manage healthcare costs and utilization.
- Participate in weekly care coordination meetings with case management and contribute to patient care plans.
- Assess and triage immediate health concerns.
- Implement medication changes and other orders as directed by providers.
- Monitor discharge lists and schedule transition of care visits.
- Conduct post-discharge hospital visits, perform medication reconciliation, and document findings.
- Make referrals to social work, pharmacy, case management, and engagement as needed.
- Provide guidance on escalated orders and results as clinically appropriate.
Qualifications:
- Active MD Compact RN license
- Minimum of 3 years of RN experience
- Previous experience as a Telephonic RN Coordinator or in RN Case Management
- Preferred clinical experience in palliative care, hospice, home health, oncology, ICU, or geriatrics
- Proficiency in Microsoft Office and Electronic Medical Records (EMR)
- Knowledge of STARS and HEDIS metrics
- Highly organized and self-directed, capable of functioning in a high-volume environment
- Strong verbal and written communication skills
