Job Purpose
The Employment Readiness Case Manager offers an opportunity to make a meaningful impact on clients' lives by helping them achieve employment stability and self-sufficiency. This role plays a crucial part within the Operation Home and Continuum of Care programs, combining employment navigation responsibilities with critical support to the case management team. The Employment Readiness Case Manager assists clients in securing stable employment while addressing broader needs such as peer mediation, case conferencing, budget planning, and workforce development.
Responsibilities
- Identify and secure suitable employment opportunities for clients.
- Build and maintain relationships with employers and job training providers.
- Provide ongoing support to clients for employment stability.
- Collaborate with case managers to address complex client issues through case conferences.
- Empower clients to make informed decisions and regain control of their employment situation.
- Connect clients with resources such as food assistance, legal services, and transportation.
- Educate clients on employee rights, workplace responsibilities, and conflict resolution.
- Assist clients in developing and maintaining budget plans for financial stability.
- Provide guidance on financial literacy, including budgeting, saving, and debt management.
- Assess clients' employment skills, interests, and goals.
- Offer job readiness support, including resume building, job search assistance, and interview preparation.
- Connect clients with job training and workforce development programs.
- Maintain accurate client records, including case notes, assessments, and progress reports.
- Prepare and submit required program reports and data.
