The Kidney Care Center, located in Hobbs, NM, is a 16-chair hemodialysis center. We also offer Peritoneal Dialysis for patients.

We are seeking a highly motivated and dedicated social worker to join our dialysis team in New Mexico. The ideal candidate will have experience in providing social work services to patients with end-stage renal disease and will possess excellent communication and interpersonal skills.
Job Description
How you will change lives
As a social worker at The Kidney Care Center, you will be an integral part of a cross-functional
team, working with healthcare providers to facilitate quality care for patients living with kidney disease.
What you will be doing
- Provide emotional support and counseling to patients and their families
- Assess patient and family psychosocial needs and develop care plans
- Coordinate patient care with other healthcare professionals
- Advocate for patients and their families in accessing community resources
- Participate in interdisciplinary team meetings and patient care conferences
- Maintain accurate and up-to-date patient records
Qualifications/Skills:
- Master's degree in Social Work from an accredited institution
- Current state licensure as a Social Worker required
- Experience in providing social work services to patients with end-stage renal disease preferred
- Knowledge of Medicare and Medicaid regulations and policies related to dialysis
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational skills
- Bilingual- Spanish speaking preferred
