Charge Nurse Case Manager – Registered Nurse (RN) – Assisted Living

Location: New York

POSITION SUMMARY:

The Charge Nurse Case Manager is responsible for providing oversight, in conjunction with the clinical leadership and Executive Director (ED), for the certification programs in the State of New York:

  • The Enhanced Assisted Living Certification (EALR)

  • The Special Needs Assisted Living Program (SNALR)

The Charge Nurse Case Manager will ensure that:

  • Resident care is delivered per each resident’s Individual Service Plan.

  • Care adheres to all federal, state, and local regulations.

  • Residents’ physical, emotional, and social needs are met.

This position involves case management responsibilities for residents within the programs and oversight of care partners and nursing staff to ensure individualized, safe, and high-quality care.

ESSENTIAL DUTIES:

The following duties are essential to this position. Other duties may be assigned as necessary.

Resident Care & Compliance:

  • Ensure that SNALR and EALR programs provide an environment that encourages independence and aging in place.

  • Maintain documentation, progress notes, and investigations in compliance with Department of Health guidelines.

  • Develop, implement, evaluate, and direct case management activities.

  • Formulate and oversee Individualized Service Plans to ensure residents’ needs are met by care partners and nursing staff.

  • Provide services in accordance with NY state regulations for EALR and SNALR.

  • Supervise Care Partners working with SNALR and EALR residents, ensuring proper training and care delivery.

  • Conduct annual performance assessments, including direct observations.

  • Participate in quality assurance and community prevention activities, including exposure risk assessment and PPE training.

Admissions & Assessments:

  • Screen and evaluate resident admissions and re-admissions to ensure appropriate placement.

  • Ensure staffing meets NYS regulatory guidelines and accommodates resident acuity needs.

  • Collaborate with Connections staff and clinical leadership to provide dementia-specific programming for SNALR residents.

  • Maintain and review resident progress notes to ensure accuracy and compliance with enhanced special needs services.

  • Facilitate appropriate transitions for residents exceeding retention standards.

Training & Development:

  • Train HHA and LPN staff on EALR services, including:

    • Protective equipment usage, infection control, and DME safety.

    • Resident-specific training for individualized and effective care.

    • Completion of 12 hours of annual in-service training per NY state regulations.

  • Ensure proper documentation of services provided and update training materials as needed.

Case Management & Quality Assurance:

  • Address all aspects of resident well-being: social, family, financial, medical, mental health, and dietary needs.

  • Conduct regular audits of documentation, physician’s orders, and individual service plans.

  • Work closely with the nursing care team to maintain regulatory records, census updates, and incident reports.

  • Communicate with families and responsible parties regarding residents’ status and changes in care.

  • Ensure compliance with admission and retention criteria, supporting appropriate placement decisions.

  • Assist with Community Preventionist initiatives, including training on PPE, sharps safety, and exposure risk mitigation.

REQUIREMENTS:

  • Registered Nurse (RN) with a current license in good standing in New York.

  • Minimum of three years’ experience working with an adult population in assisted living, long-term care, or a related setting.

  • Experience in dementia-specific care and geriatric case management preferred.

  • Strong assessment skills and ability to make sound clinical judgments.

  • Experience collaborating with hospice, home health, pharmacy consultants, and other third-party providers.

  • Ability to work independently, adapt to change, and manage multiple responsibilities effectively.

The salary range for this position is $69,377 to $110,823.
