Position Summary:
The Physical Therapist (PT) provides direct rehabilitation care in physical therapy to patients upon physician referral, adhering to all applicable laws, regulations, and Life Care standards.
Requirements:
- Graduate of an accredited physical therapy program with a BSPT, MSPT, or DPT degree (MSPT or DPT required for 2003 or later graduates)
- Current registration/licensure in the applicable state; must maintain an active license in good standing throughout employment
- One year of experience in post-acute care or a related setting preferred
- CPR certification upon hire or obtained during orientation; must remain current during employment
Specific Job Responsibilities:
- Participates in community health matters and projects as appropriate
- Proficient in Microsoft Word, Excel, and email
- Liaisons with patients, families, support departments, etc., to plan for patient needs
- Demonstrates good body mechanics at all times
- Makes independent decisions when circumstances warrant such action
- Knowledgeable of practices and procedures, as well as laws, regulations, and guidelines governing functions in the post-acute care facility
- Implements and interprets departmental programs, goals, objectives, policies, and procedures
- Performs competently in all areas, including patient rights, safety, and sanitation
- Maintains professional working relationships with associates, vendors, etc.
- Maintains confidentiality of proprietary and confidential information
- Understands and follows company policies, including harassment and compliance procedures
- Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other compliance training
Essential Functions:
- Follows physical therapy treatment plans for patients
- Establishes, assesses, and modifies realistic, measurable, timely, and functional goals
- Oversees and evaluates care given by PTAs, Rehab Aides, and PT and PTA students
- Charts appropriately and timely
- Utilizes therapy software accurately
- Exhibits excellent customer service and a positive attitude towards patients
- Assists in patient evacuation
- Demonstrates dependable, regular attendance
- Concentrates and uses reasoning skills and good judgment
- Communicates effectively on an interdisciplinary team
- Sits, stands, bends, lifts, pushes, pulls, stoops, walks, reaches, and moves intermittently during working hours
