We are seeking an experienced nurse to join our team as a Care Management Nurse, MDS Nurse. If you are organized, efficient, and able to manage your own work with autonomy, this role may be the perfect fit.

Responsibilities:
- Conduct RAI assessments and care plan coordination for assigned residents.
- Collaborate with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff, to discuss resident care needs.
- Review information from hospitals, consults, and outside agencies to inform assessment and care planning.
- Identify and initiate significant changes to MDS assessments.
- Participate in utilization review meetings and maintain MDS assessments according to Medicare schedules.
- Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience.
- AANC certification (RAC-CT) is a plus.
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
- Experience as an MDS Nurse.
