We are seeking an experienced nurse to join our team as a Care Management Nurse, MDS Nurse. If you want to remain clinically involved in patient care without being a bedside nurse, this role may be ideal for you.

Responsibilities:
- Complete MDS, CAA's, and care plans within regulated time frames.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Review information from hospitals, consults, and outside agencies, incorporating it into assessments and care planning.
- Coordinate, identify, and/or initiate significant change in MDS.
- Maintain MDS assessments per Medicare schedule and monitor skilled board for Medicare days and PDPM utilization in the absence of the Care Management Coordinator.
- Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience.
- AANC certification (RAC-CT) is a plus.
- Familiarity with the Resident Assessment Instrument (RAI) process, including Prospective Payment Process (PPS), is strongly preferred.
- Experience as an MDS Nurse.
Estimated Salary: $65,000 - $85,000 per year, depending on experience and location.
