The Laurels of Athens is seeking a skilled Nurse Care Manager to join our team. As an MDS Nurse, you will be responsible for conducting assessments and care plan coordination for residents assigned to your caseload. Your expertise in the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS), will enable you to accurately identify resident needs and develop effective care plans.
About the Role:
This position requires a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with experience as an MDS Nurse. AANC certification (RAC-CT) is highly desirable. You should have strong communication skills, the ability to work autonomously, and a commitment to delivering high-quality patient care. The ideal candidate will be well-organized, efficient, and able to prioritize tasks effectively.
Responsibilities:
- Complete the MDS, CAA's, and care plans within regulated time frames
- Assess residents through physical assessment, interview, and chart review
- Discuss resident care needs with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activity staff
- Review information from hospitals, consultants, and outside agencies, incorporating this data into assessments and care planning
- Coordinate significant changes to MDS records and conduct utilization review meetings as needed
- Maintain up-to-date knowledge of American Association of Nursing Assessment Coordinators (AANAC) requirements
