Responsibilities:
- We're seeking a skilled Care Management Nurse, MDS Nurse who can work effectively within regulated time frames to complete the MDS, CAA's and care plans.
- You'll conduct thorough assessments of residents through physical evaluation, interviews, and chart reviews.
- As part of our team, you'll work closely with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff, to discuss resident care needs.
- You'll stay up-to-date on information from hospitals, consults, and outside agencies, and incorporate this into your assessments and care planning.
- You'll coordinate and initiate significant changes to MDS assessments as needed.
- You'll prepare for PPS meetings, maintaining MDS assessments according to Medicare schedules, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
Requirements:
- We require a Registered Nurse, RN or Licensed Practical Nurse with AANC certification preferred (RAC-CT).
- You should have knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS), with experience as an MDS Nurse.
