We're looking for a compassionate and dedicated LPN to join our team at The Laurels of Carson City!
As a nurse care coordinator, you will be responsible for developing and implementing individualized plans of care, supervising CNAs and other staff members, and ensuring seamless coordination between departments. You will also be responsible for communicating effectively with guests, family members, and other healthcare professionals to ensure comprehensive care.
- Develop and maintain accurate and up-to-date records of guest's treatment plans, progress notes, and medications.
- Communicate effectively with guests, family members, and other healthcare professionals to ensure comprehensive care.
- Participate in ongoing quality improvement initiatives to identify areas for improvement and implement changes to enhance the delivery of care.
Requirements
- 1-3 years of experience in a long-term care setting preferred
- Current Licensed Practical Nurse (LPN) licensure in the state
- CPR certification
