Job Title: MDS Coordinator
Summary
The MDS Coordinator provides oversight of the Resident Assessment Instrument (RAI) process, conducting assessments and care plan coordination for residents. They supervise the Care Management Nurse, MDS Nurse, and ensure seamless collaboration with care teams.
Key Responsibilities
- Assess resident needs through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
- Review information from hospitals, consults, and outside agencies to inform assessment and care planning.
- Maintain accurate records and update information as necessary.
- Conduct Prospective Payment Process (PPS) meetings and maintain PPS boards for monitoring Medicare days and RUGs utilization.
Requirements
- Knowledge of the Resident Assessment Instrument (RAI) process and principles of Prospective Payment Process (PPS).
- Current certification in American Association of Nursing Assessment Coordinators (AANAC) requirements.
