This is an exciting opportunity for an experienced MDS nurse to join our team. As an MDS Coordinator, you will oversee the RAI process, conduct assessments, and coordinate care plans for our guests.
Key Responsibilities
- Complete MDS, CAA's, and care plans within regulated time frames.
- Coordinate scheduling the RAI process with the interdisciplinary team.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Review information from hospitals, consults, and outside agencies, and use this information in the completion of the assessment and care planning.
- Coordinate, identify, and/or initiate significant change MDS.'
- Maintain current knowledge with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure required.
- AANC certification (RAC-CT) a plus.
- Knowledge of Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS) preferred.
- Experience as an MDS Nurse required.
Estimated Salary: $60,000 - $80,000 per year
