Job Responsibilities
- Develop and implement individualized care plans that meet the unique needs of each guest
- Collaborate with other healthcare professionals to ensure seamless care coordination and delivery
- Provide education and support to guests and their families on health-related topics and disease management
- Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction
- Maintain accurate and detailed records of guest interactions, care plans, and treatment outcomes
Requirements
- Current Licensed Practical Nurse (LPN) licensure in the state
- 1-3 years of experience in a long-term care setting preferred
- CPR certification
