Overview
We are seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a MDS Coordinator at The Laurels of Greentree Ridge. As an MDS Coordinator, you will provide oversight of the RAI process and conduct assessments and care plan coordination for guests.
The ideal candidate will have experience as an MDS Nurse and possess knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS). AANC certification is a plus, and RAC-CT is preferred.
Responsibilities
- Completes the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
- Coordinates scheduling of the RAI process with the interdisciplinary team.
- Assesses residents through physical assessment, interview, and chart review.
- Discusses resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Reviews information from hospitals, consults, and outside agencies and uses this information in the completion of the assessment and care planning.
- Coordinates, identifies, and/or initiates significant changes to the MDS.
- Maintains current knowledge of American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN)
- AANC certification a plus; RAC-CT preferred
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS)
- Experience as an MDS Nurse
