**Responsibilities:**
- Develop and implement individualized plans of care for each guest.
- Contribute to the assessment process (MDS/CAA's) and participate in interdisciplinary care planning.
- Assess the health status of guests and notify physicians of changes in condition.
**Qualifications:*
- 1-3 years of experience in a long-term care setting preferred.
- Current LPN or RN licensure in the state.
- CPR certification.
