About the Role
We are seeking a skilled Registered Nurse (RN) to join our team as a Care Management Nurse, MDS. In this role, you will work closely with our interdisciplinary team to assess resident needs and develop comprehensive care plans.
Responsibilities
- Complete accurate and timely MDS assessments and care plans in accordance with regulatory requirements
- Conduct thorough physical assessments, interviews, and chart reviews to identify resident needs
- Collaborate with healthcare professionals, including physicians, nurses, social workers, therapists, and dietitians, to discuss resident care needs
- Review information from hospitals, consultants, and outside agencies to inform assessment and care planning decisions
Requirements
- Current RN licensure in the state of practice
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS)
- Experience as an MDS Nurse or similar role
