We are seeking an experienced RN to join our team as a skilled MDS Care Coordinator.
As a vital member of our interdisciplinary team, you will oversee the Resident Assessment Instrument (RAI) process and conduct thorough assessments and care plan coordination for our guests. Your expertise will ensure seamless collaboration with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
In this role, you will be responsible for completing accurate and timely MDS, CAA's, and care plans within regulated timeframes. You will also coordinate scheduling the RAI process and assess residents through physical examination, interviews, and chart reviews.
