Job Description
The MDS Coordinator plays a vital role in ensuring the highest standards of care for our residents. This position requires an experienced nurse who can provide oversight of the RAI process and conduct assessments and care plan coordination.
Key Responsibilities
- Complete MDS, CAA's, and care plans within regulated time frames
- Coordinate scheduling the RAI process with the interdisciplinary team
- Assess residents through physical assessment, interview, and chart review
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
- Review information from hospital, consults, and outside agencies and use such information in the completion of the assessment and care planning
- Coordinate, identify, and/or initiate significant change MDS'
- Conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintain PPS board for monitoring of Medicare days and RUGs utilization
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
- AANC certification (RAC-CT) is a plus
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS)
- Experience as an MDS Nurse
