Job Title: MDS Coordinator
About the Role:
The MDS Coordinator provides oversight of the Resident Assessment Instrument (RAI) process and conducts assessments and care plan coordination for residents. They supervise the Care Management Nurse, MDS Nurse.

Key Responsibilities:
- Assess residents through physical assessment, interview, and chart review
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
- Review information from hospitals, consults, and outside agencies to inform assessment and care planning
- Maintain PPS meetings and MDS assessments per Medicare schedule
- Monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator
Requirements:
- Knowledge of the RAI process, including Prospective Payment Process (PPS) principles
