What are the responsibilities and job description for the Resident Assessment Director position at The Laurels of Middletown?
Responsibilities:
- Completes the MDS, CAA's and care plans within regulated time frames
- Coordinates scheduling the RAI process with the interdisciplinary team
- Assesses resident through physical assessment, interview and chart review
- Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning
This role also involves discussing resident care needs with care givers, identifying and/or initiating significant change MDS', conducting PPS meetings and maintaining PPS board for monitoring of Medicare days and RUGs utilization.
