Key Responsibilities
- Complete MDS, CAA's, and care plans within regulated time frames
- Coordinate RAI process scheduling with the interdisciplinary team
- Assess residents through physical examination, interview, and chart review
- Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff
- Review information from hospitals, consults, and outside agencies, incorporating it into assessment and care planning
- Identify and initiate significant changes to MDS
- Conduct PPS meetings, maintaining MDS assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator
- Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements
Required Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
- AANC certification (RAC-CT) preferred
- Familiarity with the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS) principles
- Experience as an MDS Nurse
