Responsibilities
- Complete MDS, CAA's, and care plans within regulated time frames.
- Coordinate scheduling the RAI process with the interdisciplinary team.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Review information from hospitals, consults, and outside agencies, using such information in the completion of assessments and care planning.
- Coordinate, identify, and/or initiate significant change MDS's.
- Maintain PPS meetings, maintaining MDS assessments per Medicare schedule and monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure.
- AANC certification a plus, RAC-CT.
- Knowledge of the Resident Assessment Instrument (RAI) process, including principles of the Prospective Payment Process (PPS), strongly preferred.
- Experience as an MDS Nurse.
