About the Role
This is an opportunity to work as a Medication Resident Assessment Coordinator (MDS) at The Laurels of Norworth, where you will play a vital role in assessing resident needs and developing comprehensive care plans.
Main Responsibilities
- Conduct thorough physical assessments of residents, gathering information through interviews and chart reviews.
- Collaborate with interdisciplinary teams, including physicians, nurses, social services, therapy, dietary, and activity staff, to discuss resident care needs.
- Review information from hospitals, consults, and outside agencies to inform assessment and care planning.
- Maintain accurate records and PPS boards, ensuring compliance with Medicare schedules and RUGs utilization.
Requirements
- Experience as a nurse with strong clinical skills.
- Ability to work autonomously and manage own workload efficiently.
- Strong organizational and time management skills.
- Familiarity with the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
