Job Summary
We are currently seeking an experienced Registered Nurse to join our team as a Nurse Care Manager. In this role, you will be responsible for planning, coordinating, and providing nursing care services to our guests. This includes assessing their health needs, developing plans of care, and implementing nursing interventions to promote optimal outcomes. As a key member of our interdisciplinary team, you will work closely with physicians, therapists, and other healthcare professionals to ensure seamless care delivery.
Key Responsibilities
* Assess the health needs of guests and develop individualized plans of care
* Provide direct nursing care to guests, including administering medications, wound care, and vital sign monitoring
* Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
* Communicate effectively with guests, families, and healthcare professionals to ensure coordinated care delivery
Requirements
* Current RN licensure in the state of practice
* 1-3 years of experience in a long-term care setting preferred
