About the Role
As a MDS Nurse at The Laurels of Steubenville, you will play a critical role in ensuring the highest standards of care for our residents. You will work closely with our interdisciplinary team to assess and coordinate resident care, utilizing your expertise in the RAI process and PPS to drive positive outcomes.
Key Responsibilities:
- Assessment and Care Planning: Conduct thorough assessments of residents, identifying their unique care needs and developing effective care plans to address these needs.
- Care Coordination: Collaborate with caregivers, including physicians, nurses, social workers, therapists, dietitians, and activities staff to ensure seamless delivery of care.
- PPS Compliance: Maintain accurate and timely MDS assessments, ensuring compliance with Medicare regulations and optimal RUGs utilization.
- Team Collaboration: Work effectively with our care management team to identify and implement best practices in care delivery.
Requirements:
- Licensure: Current LPN licensure.
- Certification: AANC certification (RAC-CT) a plus.
- Education: Strong knowledge of the RAI process and PPS principles.
- Experience: Prior experience as an MDS Nurse.
