We are seeking a dedicated individual to fill the position of Social Services Designee/Discharge Planner at The Laurels of Steubenville.
Job Summary
The Social Services Designee/Discharge Planner plays a vital role in assisting and coordinating with the facility's Social Service Department to streamline the discharge planning process for residents.
Responsibilities
- Meet with residents upon admission and provide support throughout their stay until discharge.
- Attend 72-hour care plan meetings to ensure effective care coordination.
- Verify contact information on resident face sheets.
- Order durable medical equipment as needed, in consultation with therapy and nursing staff.
- Arrange home health care services with the resident's preferred provider.
- Coordinate IV home infusion services, wound vacs, and other necessary supplies.
- Provide nursing education to proposed caregivers.
- Oversee overall discharge coordination, including attending care conferences and Medicare meetings.
- Ensure completion of Post Discharge Plan of Care and Summary.
Requirements
- High school diploma or equivalent.
