Senior Social Worker

Responsibilities:
- Conduct admissions and social services evaluations, provide referrals, and maintain communication with residents and families.
- Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and preplanning arrangements.
- Provide discharge planning services, including arranging intake for home care services, transfer arrangements to other facilities, and working with outside agencies towards safe discharges.
- Offer counseling services and psychiatric medication management as needed.
- Maintain accurate and timely documentation in MDS care plans.
Qualifications:
- Current social work licensure in the state.
- Bachelor's degree in social work required; master's degree preferred.
- One year of supervised social work experience in a healthcare setting is preferred.
