Responsibilities:
The Social Services Designee - Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for discharge planning. Reports to the Social Services Director
- Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
- Attend the 72 hour care plan meeting
- Ordering equipment for the community setting
- Arranging home health nursing for after discharge
- Coordinating the medications for community use
- Scheduling IV home services if necessary
- Communicate with external case managers, as needed
- Coordinate the entire discharge, with IDT involvement
- Attending care conferences for sub-acute residents/guests
- Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
Education and/or Experience:
High school diploma/GED
