Job Summary:

The Wraparound Facilitator plays a pivotal role in the Wraparound process, ensuring fidelity and maintaining hope for families to achieve a better future. They initiate parent and youth engagement, facilitate Child and Family Team (CFT) formation, and develop safety plans and initial individualized Plan of Care (POC). The Facilitator consistently demonstrates a non-judgmental, strength-based approach, coordinating and refining plans based on family needs and preferences. They provide leadership within the team, fostering collaboration and integrating community resources to support family goals.

Key function, accountabilities, and related duties:

  • Develop and implement Plan of Care (POC) in collaboration with the Family Care Team (FCT) and the Family to ensure voice and choice is integrated.
  • Report directly to the Wraparound Manager in the assigned area.
  • Initiate parent and youth engagement, facilitate CFT formation, and develop initial safety plans and individualized Plan of Care (POC).
  • Explain the Wraparound process and its principles, ensuring family understanding and participation.
  • Identify immediate safety needs and facilitate safety planning discussions with families.
  • Collaborate with parents, youth, and identified team members to prioritize and address family needs.
  • Mediate conflicts during team meetings and facilitate consensus-building processes.
  • Coordinate activities outlined in the care plan, ensuring accountability and progress tracking.
  • Review and revise care plans based on ongoing evaluation and CANS assessments.
  • Facilitate the use of flexible resources and manage documentation and approvals.
  • Support crisis intervention efforts, coaching families through safety plans or convening emergency CFT meetings.
  • Maintain effective communication among team members and document progress accordingly.
  • Utilize tools such as CANS and WFI-EZ to track and improve CFT processes.
  • Other duties as assigned. Qualifications
  • Bachelor's degree in social work, Psychology, Counseling, or related field preferred.
  • Experience in trauma-informed care, assessment, and developing care plans.
  • Strong organizational and communication skills, with the ability to facilitate group processes effectively.
  • Familiarity with community resources and crisis intervention strategies.
  • Engaging, empathetic, and non-judgmental.
  • Optimistic, strength-based, and creative in problem-solving.
  • Strong organizational skills and effective time management.
  • Demonstrates cultural humility, understands diversity, equity, and inclusion issues.
  • Proactive communicator and clear, concise writer.
  • Ability to facilitate group processes and integrate divergent perspectives.
  • Self-aware and capable of self-care management.
  • Certified to complete CANS assessments.

Education & Experience:

  • Master’s degree in social work or other related discipline. Bachelor's Degree with extensive experience may be considered.
  • Experience in wraparound or other clinical services
  • Management experience required
  • ·At least two years’ prior experience working in the social services field

Pay:

  • $65,000.00 - $70,000.00 per year
