Position Summary
The Case Manager plays a vital role in supporting individuals receiving services such as Meaningful Day, Employment, Personal Supports, and Behavioral Support Services. Using a person-centered approach, the Case Manager develops Service Implementation Plans (SIPs) with meaningful, measurable, and individualized goals, while ensuring consistent follow-through and quality monitoring.
This position involves close coordination with Coordination of Community Services (CCS), Direct Support Professionals (DSPs), caregivers, and other team members to ensure that each participant’s goals and supports are aligned and evolving based on their needs.
Key Responsibilities
Coordinate with DDA CCS to schedule and facilitate initial, annual, and revised Person-Centered Plan (PCP) meetings.
Conduct 90-day reviews to assess progress toward goals and recommend updates or adjustments as needed.
Maintain accurate, up-to-date participant records and documentation.
Provide responsive, quality case management services and connect participants with appropriate community resources.
Communicate monthly with participants and caregivers regarding progress and emerging needs.
Serve as the department's primary Incident Report writer, ensuring timely and accurate documentation.
Represent The League at transition and resource fairs to promote our programs and services.
Additional Responsibilities
Support the Intake Coordinator in reviewing and processing new participant applications.
Take on additional duties as assigned by management.

Qualifications
Bachelor’s Degree in a related field preferred.
High School Diploma required with a minimum of 3 years’ experience in case management and/or working with individuals with disabilities.
Familiarity with DDA and DORS regulations preferred or willingness to learn quickly.
Proficiency in Microsoft Word, Excel, and Outlook.
Strong written and verbal communication skills.
Excellent organizational skills and the ability to work independently and manage multiple responsibilities.
