What are the responsibilities and job description for the Family Nurse Practitioner position at The Leap Network, LLC?
Established multi-specialty healthcare leader is excited to add part time and full-time (30 hrs) Family Nurse Practitioners passionate about community health and providing quality, accessible care to vulnerable and underserved populations where English may or may not be their first language.
The FNP will provide comprehensive Primary Care to patients within an outpatient environment. Also will deliver direct primary care and preventative services to assigned children, adolescent, and adult patients. They will coordinate the care of patients under the supervision of a Board-Certified Physician. In addition, the FNP acts as a member of the clinical team and works collaboratively with other departments in providing patient care, health education and counseling to patients.
PATIENT POPULATION:
- 33% English speaking
- 33% Portuguese speaking (Brazilian)
- 33% Spanish speaking
PROJECTS:
- Assist with the transition to new EMR in 2026
- Provide support to growing Medication Assisted Treatment (MAT)/OBAT program
- Attends trainings, weekly Provider and Team meetings, and collaborates on practice, administrative, and quality improvement projects, as assigned by Clinical Director
