The Lilac at Bayview - a 120 bed Skilled Nursing Facility located in St Augustine, Florida is currently seeking a Social Services Assistant. We are a growing company in need of an energetic, motivated, and talented individual to join our team!
As the Social Services Assistant (SSA) you will assist the Director of Social Services in the ongoing efforts to facilitate and improve each resident’s social, emotional and psychological well-being. The ideal Social Services Assistant will be able to find new ways to add value daily, by easing the workload of the department and also the Director of Social Services.
TEAMWORK ENVIRONMENT
SSA Job Summary & Qualifications:
- Assist in collecting data for the development of discharge plans,
- Collect data as it relates to the MDS and Care Plans for new admits
- Assist residents and families with adjustment to changes, including room changes.
- Document progress notes related to care plans
- Minimum (1) year experience in social work, preferably in an LTC, SNF, or hospital facility.
- Ability to communicate at all levels of organization
- Polished Communicator, Customer Service Oriented
- Ability to work with minimal supervision, take initiative and make independent decisions.
- Approachable, flexible and adaptable to change

Pay: Up to $23.00 per hour
Expected hours: 12 – 30 per week
