Position Summary: |
The Licensed Social Worker, under the Violence Intervention Program of Trinity Health Mid Atlantic (THMA), will provide direct services to individuals affected by violence. The VIP Program will be implemented in the three THMA PA hospitals: Mercy Fitzgerald Hospital, Nazareth Hospital and St. Mary's Medical Center, along with THMA outpatient support. The Licensed Social Worker will be based at one of the designated hospital locations. The social worker will assess client needs, develop and implement treatment plans, and provide ongoing evaluation of services. Reporting directly to the Violence Intervention Clinical Supervisor, the successful candidate will have a strong commitment to public service and a passion for helping clients overcome the challenges associated with trauma and violence. |
Duties & Responsibilities |
· Provide direct clinical services to clients, including assessments, counseling and crises intervention. · Develop, implement and evaluate individual treatment plans based on client needs. · Participate in multidisciplinary team meetings and case discussions. · Enter direct and indirect services into Simple Practice and documentation into THMA EPIC system within 2-3 business days from the service. · Maintain additional thorough and timely grant documentation according to TLC and THMA regulatory standards. · Participate in community outreach and violence prevention and intervention initiatives. · Foster and promote communication between THMA, TLC, community resources and law enforcement as needed. · Provide crisis intervention and de-escalation as needed. · Coordinate community resources to best meet client needs. · Identify clients and family members in need of additional trauma support and to provide prevention, referral and treatment services. · Provide home-based services to the families of clients as needed. · Adhere to ethical and legal standards of social work practice. · Participant in ongoing professional development and training. · Perform other duties as assigned by the Clinical Supervisor. |
Skills & Knowledge: |
· Experience providing clinical services in violence intervention and trauma-informed care · Strong understanding of the psychosocial aspects of violence and trauma · Excellent oral and written communication skills · Ability to work with diverse groups and in challenging situations · Strong organizational and case management skills · Ability to work effectively on a multidisciplinary treatment team · Knowledge of community resources and networking ability · Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook · Understanding of confidentiality and ethical guidelines in clinical services |
Education & Experience: |
Master's degree in social work. Current license required: either Licensed Social Worker (LSW) Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC). Minimum of 2 years of experience in violence intervention or trauma-informed care. |

Hours of Work: |
This is a full-time, 40 hours per week, position. This position will cover a split shift (11am-7pm or 12-8pm), due to the expected and sudden need for crisis intervention. |

Hospital-based out of designated THMA hospital location, with regular travel in the community and other hospital locations. |
Travel: |
Travel to hospital sites, patient homes, community settings and other locations. |
