PROGRAM: Clinical Services (Assessment & Evaluation)
POSITION TITLE: IM CANS Assessment/Evaluator
REPORTS TO: Director of Clinical Operations
JOB TYPE: Full-Time
The Assessment/Evaluator is responsible for completing the IM CANS and other evidence-based assessment tools as indicated by the client’s presenting problems.
RESPONSIBILITIES:
Complete IM CANS with TLOC policies and procedures (i.e., within 1 day of urgent care and within 4 days for routine care).
- Reviews and confirms initial diagnosis and treatment recommendations.
- Completes required documentation in the client’s EHR Dr. Cloud.
- Completes and submits encounter notes as required by TLOC policy (i.e., within 45 hours).
- Confers with the Director of Clinical Operations regarding questions related to differential diagnosis.
- Works closely with Intake Staff to ensure that clients are linked to appropriate services and level of care.
- Provides consultation to assigned clinical staff regarding assessment and treatment recommendations.
- Completes a Safety Plan as needed with the client and provides appropriate follow-up.
- Ensure at minimum, all clients must be educated about 24-hour Crisis Hotline.
- All clients are engaged to complete Crisis Plan.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- Must meet criteria for qualification as a QMHP under IL. Administrative Code Rule 132. LPHA preferred.
- Demonstrated knowledge of JACHO accreditation standards.
- Demonstrated knowledge of IL Administrative Rules 132, 140, and 2060.
- High degree of professionalism in dealing with diverse groups of people, including clients, staff, and others.
- Demonstrated ability to work collaboratively with other departments within TLOC.
- Ability to make appropriate, informed decisions.
- Ability to multi-task, complete a high volume of tasks and projects with little or no guidance.
- Well-developed organizational and time management skills.
- Effective verbal and written communication skills.
- Professional discretion and adherence to confidentiality.
- Must be culturally competent, empathetic, and able to interact and develop rapport with a diversified population.
