What are the responsibilities and job description for the FAMILY ADVOCATE position at The LJD Jewish Family & Community Services?
JOB SUMMARY:
Works with pre-identified at-risk children in Middle Schools who are transitioning into sixth grade. Coordinates Family Team Conference and develops a Family Success Plan in partnership with the family and the Achievement Advocate.
Minimum Qualifications:
Education: Bachelors degree in Counseling, Social Work, or related field from an accredited college or university, Masters degree preferred.
Experience: Minimum six month experience in case management or social work practice with children and families preferred
Essential Job Functions:
- All Family Advocates are expected to recruit incoming 6th grade families at their assigned school and provide intensive case management services to all families members assigned to the advocate at the school and in the home.
- Advocate for the needs of every family and make referrals to needed services.
- Monitor children, parents/caregivers enrolled in the program, including scheduled visits. and phone calls with all parties.
- Provide information and referral and coordinating/monitoring services.
- Assist parents with success plan tasks (including setting up appointments and transportation) and will staff cases with the program supervisor, school staff and program partners.
- Interact with family members and provide culturally competent services.
- Coordinate available community services to meet the needs of the family including the creation of referrals for services and follow up on if the family has engaged in services and their progress.
- Provide education for family members about available services, parenting practices and counseling.
- Work cooperatively and communicate as part of a team of advocates within JFCS and with other program partners in the school.
- Organize and conduct assigned "Family Night" events at the advocate's assigned school and help other family advocates conduct family nights at their assigned schools. FA will also participate in and help organize the AFL End of the Year Celebration.
- Maintain files on each family and document conversations, referrals made and services provided to families in ETO on a weekly basis.
- Attend and participate in staff meetings and developmental training activities.
- Complete all other duties as assigned by immediate Supervisor or AFL Manager
