The Job
- Team Leader of the Therapy Department and ensures communication between Therapy and other IDT members.
- Will directly report to the Therapy Regional Manager and will work under Facility Administration
Responsibilities
- Check Rehab Optima planner for each patient. Every planner each day. This includes check and clear out alerts from Optima “hotlist”
- Print out ARD reports for MDS as needed
- Attend daily stand up meetings and check scheduler in Optima to ensure adequate staffing
- Update therapists on admissions, readmissions, discharges, incidents, falls and accidents.
- Check therapy orders and verify payor source in PCC
- Add new patients in Nethealth and plan minutes on tracker
- Conduct Rehab Meeting and set upcoming DCs in Nethealth tracker
- Ensure trackers are set up for at least 2 weeks to 30 days in advance in Nethealth.
- Attend MCR/MCD meeting, Triple Check meeting, Care Plan meetings for patients on rehab (treating therapist can attend as well in place of DOR) and submit any weekly facility report(s) to Administration or to your Rehab Regional Manager
- Run P and L report in Nethealth to ensure financials are on track
Must haves/ Minimum Requirements
- Candidate must hold a current and active Texas State license as a Physical/ Occupational Therapist, Physical/ Occupational Therapist Assistant or Speech Language Pathologist.
- DOR experience preferred, and a minimum of 3 years of practice in the field of therapy they are licensed
- Experience in Skilled Nursing/ Longterm Care/ or Transitional healthcare as a DOR previously is preferred.
- Strong organizational and leadership skills, along with effective interpersonal and interdisciplinary communication skills.
- Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.) but must know with proficiency in Net Health Rehab as the primary Operating Software.
- Experience in health care / hospitality or other related field.
- Possess a passion for working in a retirement community with a geriatric population.
