Job Description: Registered Nurse (RN) / Licensed Practical Nurse (LPN)
Location: Adult Family Home (Ventilator and Tracheostomy Care), Vancouver, WA Employment Type: 12-Hour Shifts (7:00 AM - 7:00 PM or 7:00 PM - 7:00 AM)
We are seeking a compassionate and motivated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in an adult family home specializing in ventilator-dependent patients with tracheostomies. This position offers an excellent opportunity for new nurses or those with limited experience to gain hands-on skills in airway management, wound care, ostomy care, foley catheter care, PEG tube management, and occasional IV medication administration.
Our supportive team environment and personalized training make this a great setting to develop expertise in caring for medically complex patients while providing compassionate, high-quality care in a smaller, more intimate healthcare setting. Caregiver-to-resident ratio of 2:5
Key Responsibilities
1. Airway Management:
o Perform tracheostomy care and ensure proper functioning of ventilators.
o Manage suctioning and maintain airway patency as needed.
o Monitor respiratory status and respond promptly to any changes in condition.
2. Wound Care:
o Provide wound care, including dressing changes and monitoring for signs of infection or complications.
o Follow physician orders and best practices for wound healing.
3. Ostomy and Foley Catheter Care:
o Manage ostomy appliances, perform skin care around stomas, and address patient needs related to ostomy care.
o Perform foley catheter care, including insertion, monitoring, and troubleshooting.
4. PEG Tube Management:
o Administer feedings and medications via PEG tube as prescribed.
o Ensure cleanliness and functionality of feeding tubes and equipment.
5. Medication Administration:
o Properly administer medications using the 5 Rights of Medication Administration: right patient, right drug, right dose, right route, and right time.
o Monitor patients for medication effects and side effects, and document findings accurately.
6. IV Medication Administration (Occasional):
o Prepare and administer IV medications as needed under physician orders.
o Monitor IV sites for patency and signs of complications.
7. Patient Monitoring and Assessment:
o Conduct comprehensive assessments of patients' conditions, including regular skin assessments to monitor for pressure sores, wounds, or other skin-related issues.
o Monitor vital signs and document findings accurately.
o Follow infection prevention techniques to ensure the safety and well-being of all residents and staff.
o Report changes in patient status to the care team or physician as necessary.
8. Team Collaboration:
o Work collaboratively with caregivers as a team to ensure patient hygiene and assist with Q2 (every 2-hour) turns.
o Provide guidance and support to caregivers to ensure consistent, high-quality care.
9. Communication:
o Work closely with caregivers and other team members to provide holistic, patient-centered care.
o Communicate effectively with family members and other stakeholders regarding patient care plans.
Qualifications
· License:
o Active Washington State RN or LPN license in good standing.
o If currently licensed in Oregon, must obtain a Washington State license before starting employment.
· Experience:
o No prior experience required—new graduates and nurses with limited experience are welcome!
o Prior experience in tracheostomy or ventilator care is a plus but not required; on-the-job training will be provided.

Preferred Qualifications
· Current CPR and First Aid certification.
· Familiarity with adult family home regulations in Washington State.
