Participates in a multi-discipline case conferences. Consult with agency care providers regarding social service needs of clients and families. Location: Lowell General Hospital - 295 Varnum Avenue - Lowell, MA Hours: 40 hours days - Flexible schedule Minimum Qualifications Master's degree: MSW LCSW, or LICSW Required, or ready to sit for license 2-3 years of experience Duties & Responsibilities As a Social Worker here at Lowell General, you will: Asses selected cases within a defined patient population to evaluate for potential psychosocial barriers to quality patient outcomes and timely, safe, and appropriate discharges Work with individuals who may have substance abuse, mental health conditions, or housing instability Intervene in situations where the treatment plan can be significantly impacted by providing the patient/family with support and counseling regarding social, emotional, and/or financial issues. Work collaboratively with a healthcare team Focus on connecting patients and their families to resources in the community Assist hospital staff dealing with cases of abuse and neglect and report cases to appropriate agencies/authorities.
