About the Role
We are seeking an experienced MDS Nurse to take on a challenging role in our team. As an MDS Coordinator, you will oversee the RAI process and conduct assessments and care plan coordination for our guests.
Key Responsibilities
- Complete the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
- Coordinate scheduling of the RAI process with the interdisciplinary team.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Review information from hospitals, consultants, and outside agencies, incorporating it into the assessment and care planning process.
- Identify and/or initiate significant changes to MDS records.
- Conduct PPS meetings and maintain accurate tracking of Medicare days and RUGs utilization.
- Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Requirements
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure.
- AANC certification is desirable; RAC-CT certification is also preferred.
- Strong knowledge of the Resident Assessment Instrument (RAI) process, particularly the principles behind the Prospective Payment Process (PPS).
- Experience working as an MDS Nurse.
